I believe this question can have a ton of different answers but truly comes down to what the goals are for the piece your developing the interactive design. Designing usable products is not easy, particularly in consumer applications where the range of user characteristics (e.g., physical capabilities, usage patterns, technical skill) is so broad and variable. Good user interface design is also driven by creating an easy to use product.
Product design teams may be inexperienced with user-centered interface design methods and may be intimated by the process and unsure where to start to holistically improve an interface design. At the opposite end of the spectrum, organizations with in-place user interface and research processes may be overloaded with interpreting qualitative and quantitative data to drive actionable design decisions.
So breaking down the necessary items that need included within the interactive design from the begining is a very critical process that takes time, a great eye for design, and the ability to factor in all the goals of the project.
